We have to think that Baker’s cyst will reoccur after any aspiration and surgery is not generally recommended as the recurrence rate is high; however, we may consider surgery if all the conservative management has failed on many occasions and is causing a problem to the patient. The surgery is usually done through a posterior approach when the cyst is dissected from the surrounding tissue and excised in full.
