Anterior cruciate ligament tear is probably the most common sports injury of the knee joint. The anterior cruciate ligament is the main stabilizer of rotation movement and anterior translation of the knee joint and if ruptured it can cause significant instability in the knee joint.
The instability of the knee joint is usually divided into two: /
Static instability: This occurs when the patient's experience gives way to normal day-to-day activities including walking and changing direction
Dynamic instability: This does not occur during day-to-day activities and only occurs if the patient is practicing a sport such as running jumping or playing a contact sport.
- Unfortunately, the ACL is an unrepairable structure due to the nature of it is composition, location, and the specific tension required to act properly. ACL can be stretched before rupture, leading to loss of ligament tension, making the ligament inefficient. In my opinion, The term partial rupture of ACL is almost certainly incorrect and the ligament in most injury cases has lost its functionality even if not fully torn.
- It is recommended to reconstruct the anterior cruciate ligament following injury particularly in athletes, young, and active people to restore the stability of the knee to its maximum possible and to prevent future progressive degeneration of the knee joint. However in some cases when static instability is not present, we can accept leaving the patient alone and having conservative management with physiotherapy particularly if he does not have a very active lifestyle.
- ACL construction surgery has become a straightforward day-case procedure with a 95 % success rate, due to the advanced technique and surgeon experience. The surgery involves using the hamstring tendon as a replacement graft for the ACL. This graft is usually inserted through two tunnels within the lower part of the femur and upper part of the Tibia allowing the graft to be inserted into the center of the knee with the required tension.
Complications following ACL reconstruction are rare and almost minor including infection, blood collection within the knee, graft failure or impingement, Joint stiffness, and post-operative pain.
