What is Bunions (Hallux Valgus:

Its lateral (toward outside) deviation of the foot thumb with or without rotation and medial (toward the midline) deviation of the first metatarsal will cause medial prominence of the first metatarsal head with a soft-tissue enlargement (bunion).So it’s not a single deformity but rather a complex deformity of the first ray. It is more common in women, probably there is family history. There are two forms of Bunions namely the adult form and the juvenile form.

Risk factors of Bunions :

• Ligamentous laxity
• Rheumatoid arthritis
• Cerebral palsy
• Flat foot 
• Shoes with high heel
• Narrow toe box 

The deformity is often bilateral and is associated with callosities (a thickened and hardened part of the skin) and hammer toes deformity of the toes.

 The main complaint is the pain at the bunion and ill-fitting footwear.

How we treat:

First line treatment is shoe modification with pads to protect bunion friction and spacer between toes and orthoses for patients with flat feet or metatarsalgia.

In the severe cases we do surgical correction by soft tissue procedure and bone osteotomy (1metatarsus and the base of the thumb )with minimal invasive technique and by using the updated small implants.   

And for severe deformity in young patient with open physis(less than 16 years old) we prefer to do cuneiform (mid foot bone) osteotomy that will resolve the problem without affecting the growth of the foot.
