It’s well known as congenital talipes equinovarus. It is a common birth deformity. 50% of cases are bilateral. This common birth deformity affects both boys and girls, with a higher prevalence in boys.

Clubfoot is a complex deformity that affects the posterior (hindfoot), middle (midfoot), and anterior (forefoot) parts of the foot. In the hindfoot, there is equinus and varus, causing the foot to tilt downwards and towards the body’s inner side. In the midfoot, there is a cavus deformity, resulting in a high arch. In the forefoot, there is adduction, causing the foot to move towards the body’s inner side.

Around 20% of clubfoot cases are associated with other musculoskeletal deformities, and there is a family history in 25% of cases.

Treatment Approach

The primary treatment for clubfoot at DRHC Dubai is the Ponseti method, which has a success rate of over 90%. This technique involves serial manipulation and casting initiated as early as the first week after birth. Casting is performed weekly, with five to six casts typically required for full correction.

Additional Procedures

In most cases, a percutaneous Achilles tenotomy may be necessary during the final cast to address residual equinus. This minor procedure can be performed in the clinic under local anesthesia. Following cast removal, a removable orthotic device is worn for up to three months, transitioning to night time wear until the age of five.

Surgical Intervention

For neglected or recurrent clubfoot cases, surgical intervention may be required. Procedures such as Achilles tendon lengthening and posteromedial capsule release with tendon lengthening can yield excellent short and long-term results.
