Perineoplasty - DRHC Dubai Obstetrics and Gynecology Clinic

A plastic surgery procedure is done to correct or repair damage or defect of the vagina and puerperium or vulva.

Perineal damage can be due to:

  • Childbirth

The stretching of parturition can cause tears to the tissue or might request cutting (episiotomy). Should the woman’s birth canal prone too narrow to allow unobstructed passage of the baby more over the perineal tissue that presents either cut or tear can heal and fuse together before the suture dissolves?

  • Obesity

It can over-stretch and damage the structure of the perineum and can also stress and cause separation of the perineal muscles.

  • Sexual penetration
  • Inadequate innervation

    Either absence of nerves or too few nerves.

Indications

  • Women will report a feeling of looseness in their vulvovaginal area with decreased sexual satisfaction compared to the incidence before perineal damage.
  • Women following bariatric surgeries after losing so much of their weight will have loose vulva perineal skin and tissue.
  • Dyspareunia (Following deliveries). Painful intercourse due to episiotomy wound healing causes a very tight vagina due to over-suturing.

 

Procedure

V-shaped incision made in the posterior wall of the vagina.

Can be straight or curved or angled upwards, usually determined by the degree of vaginal tightening to be achieved.

Any existing scar tissue will be removed.

Correction may include correction of the damaged pelvic muscles and other perineal muscles.

Skin closure by subcuticular cosmetic sutures done.

Post-operative care

  1. Analgesic for 2-3 days and may continue for a week.
  2. Oral and local antibiotics for 5 days.
  3. Rest of the refrain from work for a week or less.
  4. Keep the wound clean by washing it with soap and water every time.
  5. Complete absorption of sutures will happen in 6-8 weeks.
  6. Can resume normal activities in 4 weeks.
  7. No suture removal is needed as the stitches will dissolve within 6-8 weeks.
  8. Dissolving suture can cause mild itching which can be treated by anti-histamine.
  9. A follow-up check-up with the surgeon after 1-2 weeks’ time is advised.
  10. After 4 weeks of surgery, you may do your usual activities, relaxing exercises, and lifting light weights can be permitted. However, the patient should refrain from running, soaking in the bathtub, swimming, heavy lifting, cycling, and sexual intercourse for 6-8 weeks after surgery.
