 Ankle fractures are classified according to the site of injury:

  • Type A – below the syndesmosis (the connection between the fibula and tibia bone at the level of the ankle joint)
  • Type B – at the level of the syndesmosis
  • Type C – above the level of the syndesmosis

There are also some sub-types for these fractures which indicate the stability and the need for surgical fixation.

Treatment Options

The treatment for ankle fractures depends on the type and severity of the fracture:

  • Type A Fractures: Most Type A fractures are treated conservatively with a plaster for 6 weeks.
  • Type B-1 Fractures: Fractures at the level of syndesmosis but not displaced with intact ligaments can also be treated conservatively.
  • Further B and C-type Fractures: These are usually treated surgically to maintain the length of the fibula and provide good stability for the ankle joint in the long term. This also helps to avoid complications like malunion and nonunion, and prevent osteoarthritic changes in the ankle joint.

We usually fix the lateral malleolus or distal fibula with a lag screw which holds the 2 fragments together before we apply a neutralization plate to eliminate the forces around the fracture site allowing that to heal.

Surgical Interventions

Post-operatively, we apply a splint around the ankle for approximately 10 days. After that, we either put on a full cast or an air cast, allowing the patient to partially weight-bear with the help of crutches. Most of these fractures heal within 6 weeks, after which the patient may require physiotherapy to regain their strength and full range of motion of the ankle joint.
Fractures of the medial malleolus can also occur and can be treated conservatively or surgically depending on displacement and other associated injuries. Complex injuries which involve fractures of both the medial and lateral side and even the posterior part of the ankle joint almost always require surgical fixation for superior results.
