Laparoscopic Cholecystectomy at DRHC Dubai

Cholecystectomy:

The removal of the gallbladder can be done either laparoscopic (minimally invasive) or as classical open surgery. The standard procedure is, of course, laparoscopic unless some factors, although rare, like diffused peritonitis, advanced adhesions due to previous surgeries, or multi-morbid patients.

The procedure is done as a day case surgery, unless in a morbid patient, in which case the patient can stay in the hospital for 1-2 nights

Laparoscopic Surgery Steps:

  • Performed under general anesthesia in the supine position. The patient will be draped in the usual sterile fashion.
  • A small infra or left para-umbilical incision will be made. The fascia will be, between two clamps, elevated and incised. Opening of the peritoneum and placement of an 11-mm trocar in the abdominal cavity.
  • Pneuperitoneum will be established to reach an intra-abdominal pressure between 12-14 mmHg. Two additional ports are then placed 11 mm in the epigastric area and 5 mm in the right middle upper quadrant.
  • The patient will be placed in a reversed Trendelenburg position with a slight rotation to the left. The fundus of the gallbladder will be retracted superiorly, and the neck region will also be retracted.
  • Around the base of the gallbladder, the peritoneum will be dissected using electrocautery.
  • The triangle of Calot (including cystic duct, common hepatic duct, and cystic artery) will be carefully opened. Identification of the cystic duct and cystic artery after then, placement of clips on the cystic duct and cystic artery.
  • Using scissors, the duct and artery are divided. With electrocautery, the gallbladder will be detached from the liver. With the help of Endo-bag, the gallbladder will be removed from the abdominal cavity through the infra-umbilical port side.
  • Inspection of all dissected areas, irrigation with saline, and suction. Under the vision, the removal of all ports. Deflation of the abdominal cavity. Closure of the fascia at the infra-umbilical site. Closure of the skin incisions. Infiltration of skin incisions with local anesthesia to avoid postoperative pain.
