Benign(Idiopathic) Intracranial Hypertension - Neurology Clinic Dubai DRHC

This is a type of headache syndrome that comes with visual symptoms apart from headache, but it is not a migraine.  Early diagnosis is very important because it can cause visual loss.

Common symptoms are-

  • Headache
  • Blurred vision/ double vision
  • Nausea/ vomiting
  • Black spots before eyes
  • Tunnel vision

Particularly common in young females it is characterized by normal MRI Brain and normal CSF composition on analysis, but when pressure of the CSF in the brain is measured, it is high.

Perimetry, a test to check the field of vision is used to find out any undiagnosed defects in the field of a patient’s vision

Lumbar puncture or spinal tap is a gold standard diagnostic test done for this disease which tells us the exact value of CSF pressure inside the brain. It is done after an MRI Brain has ruled out other causes for headaches

When CSF pressure in the brain is high, it exerts secondary pressure on the nerve that connects the eye to the brain, and this in turn results in blurred vision. When the CSF pressure is indeed significantly high, draining of some CSF through a spinal tap needle helps to relieve headache and visual distress immediately.

For long-term management patients, they need to take medications that can slow the rate of CSF production inside the brain. Most of the patients improve with medications, weight reduction, and spinal tap.

Perimetry and other eye investigation modalities help in follow up of such patients apart from the Neurologist’s consultation.

A special MRI based test called MR Venography sometimes demonstrates a narrow venous channel inside the brain and some researchers think this is the cause for this syndrome.

Sometimes surgical treatment is required in late stage presentation or when medical treatment fails.
