Hemospermia - Blood in the Semen - DRHC Dubai Urology Clinic

Blood in ejaculatory fluid(semen) is considered a symptom of various possible reasons. Blood in the ejaculation is often worrisome. It usually affects patients at the age 30-50. Most of the cases are not serious but reason for malignancy must be considered.

Causes of hematospermia:

  • Vesiculitis
  • Prostatitis (ca. 40%) 
  • Polyps, cysts, stones and telangiectasia.
  • Varices and carcinoma
  • Urethral and bladder: condylomas, polyps, and utricular cysts.
  • Seminal vesical stones, cysts, diverticulum, and carcinoma.
  • Infection:
  • From my experience, I believe that bacterial infection is one of the most important factors.
  • Different types of bacteria.
    The following are considered more infectious: Escherichia coli, gonococci, rare due to Mycobacterium tuberculosis.
  • Any procedure on the prostate such as biopsy or any procedures affecting the prostate, transrectal.
  • Epididymo-orchitis, testicular tumor.
  • Without underlying reason, it may reach to (ca. 30%).

I believe that the most common cause is due to infection (under 40 years old, it reaches up to 16%, above 40 years old, it reaches up to 11%).

More than 40 typical microorganisms are responsible for urinary tract infections such as Escherichia coli, Proteus mirabilis, Klebsiella pneumoniae, and Enterobacter.

Blood in sperm is not considered a major symptom of chronic prostatitis but statistically, we found that it reached to 7.1% of the cases of accompanying infection of testes and epididymitis. It also reached 26% in some rare diseases like in tuberculosis disease with cases of prostate involvement.

Some bacterial infections can be more specific such as Neisseria gonorrhoeae, Treponema pallidum, and Cytomegalovirus.

Treatment for Hemospermia:

Treatment for blood in semen depends on many factors. Most cases need special examination and investigation. In general, we classify the patients into two groups:

  • Under 40 years of age, there is usually no serious underlying disease. 
  • Age over 40 years, there is repetitive or persisting hematospermia.

We have to take it seriously in thinking about cancer. These equations and signs are highly considered. Sometimes even with a family history of prostate cancer, elevated PSA, hematuria, and all other investigations, the reason for hemospermia remains unclear.
