Congenital Dislocation of the Knee - DRHC Dubai Pediatric Orthopedic Clinic

Deformity of the knee can occur in children with myelomeningocele, arthrogryposis, Larsen's syndrome.

The etiology is quadricep tendon contracture(the anterior muscle of the thigh), anterior subluxation of hamstring tendon(posterior muscle of the thigh), tight collateral knee ligament, absent suprapatellar pouch(space above the patella).

This condition is often associated with developmental dysplasia of the hip(50% of cases), clubfoot, metatarsus adductus.

Treatment

  • Most cases can be treated non operatively and if there is associated dysplasia or dislocation of the hip we address the knee first.
  • For the mild cases( range of passive flexion more than 90 degrees ) we do reduction with manual manipulation and casting repeated weekly .
  • For moderate and sever cases or if we cant  gain 30° of flexion after 3 months of casting and in recurrent cases we do percutaneous quadriceps recession, quadriceps tendon lengthening, anterior joint capsule release, hamstring tendon posterior transposition, and collateral ligaments mobilization.
  • After surgery we put cast for 3-4 weeks.
  • The goal of surgery is to obtain more than 90 degrees of flexion .
